Expectations & Operations

This section introduces important GulfSouth Autism Center policies and expectations, including attendance, daily supplies, transportation, and other information about your child’s participation in the program.

You’ll have an opportunity to review and acknowledge each item as you go.

Acknowledgements & Releases

Here you’ll review the consents, acknowledgements, and permissions needed for your child’s assessment and treatment.

Where appropriate, you will be prompted to indicate your individual preferences before acknowledging and signing.

Disclosures & Protections

This section explains important rights, privacy protections, and policies designed to protect your child, your family, and others participating in GulfSouth Autism Center programs.

You will be prompted to review each item carefully before acknowledging or signing.

Clinical Intake & Follow-Up

The final section gathers additional information through Speech, Occupational Therapy, Feeding, and supplemental screening forms to help our clinical team identify areas where further evaluation or support may be beneficial.

Once all five sections of the registration packet are complete, you’ll be prompted to review and submit your registration.
